PRIVACY & COOKIES

Your information.
Handled thoughtfully.

Information about this public website and your initial enquiry.

Who is responsible for your information?

The data controller is Victoria Chadwick, sole trader trading as Ardsley Foot Care. Victoria is the Foot Health Practitioner who provides the service. For questions about your information, email hello@ardsleyfootcare.co.uk or call 07361 591 007.

What this website collects

The short enquiry form asks for basic contact and visit information. The separate New Patient Details form can collect patient contact details, foot concerns, relevant health information, mobility/access information, consent choices and up to 10 files or photographs that you choose to provide. Because some of this is health information, it is treated as sensitive information and should only be submitted when relevant to the patient’s foot care.

Providing these details is voluntary. Without the required contact information we cannot respond through the form. You may telephone instead.

Why the information is used

We use enquiry details to respond, check whether a visit can be arranged and take steps at your request before a potential booking. Where you are an organiser or enquire on behalf of someone else, our legitimate interest is responding to your request and arranging appropriate contact. Our legitimate interest in protecting the website also supports proportionate security checks.

The form does not subscribe you to marketing. It does not make a treatment decision or confirm an appointment automatically.

How an enquiry reaches us

The form sends an email to the business mailbox using the website host’s mail service. Hosting and email providers process information to deliver and protect the service. The website application does not keep a separate database of submitted enquiry content. Emails, email-provider records and hosting logs are separate from this application.

Security information and cookies

The enquiry form uses a session cookie called AFCSESSID to protect submissions. It is a session cookie rather than a marketing cookie. The server uses a pseudonymous rate-limit identifier and submission timestamps to help prevent abuse; the application deletes expired rate-limit records after 24 hours when its cleanup process runs.

This website contains no advertising pixels, analytics scripts, embedded maps or remotely loaded fonts. The original logo is fetched and cached by the website server. If that image cannot be served locally, your browser may request the original logo from its image hosting provider, which receives normal connection information such as your IP address.

How long information is kept

Enquiry correspondence is kept while your request and any related follow-up are being dealt with. The retention decision also takes account of whether the enquiry becomes a booking, whether there is an unresolved complaint or dispute, and any record needed to meet a legal obligation. Information that is no longer needed for those purposes should be deleted. Relevant information for an agreed appointment is handled under the separate patient-record arrangements explained to you before care.

Health information and care records

The website sends a PDF summary of a New Patient Details submission, together with any supplied files, to the configured Ardsley Foot Care email account. The website code does not intentionally save a separate copy of the submitted patient content after the email has been accepted by the hosting mail service; normal hosting, email-provider and security logs may still exist. Patient information retained after receipt must be managed under Ardsley Foot Care’s clinical and privacy procedures. Do not submit information about another person unless you are authorised to do so.

Your rights

Depending on the circumstances, you can ask to access or correct your information, request erasure or restriction, and ask about portability. These rights are not absolute and some information may need to be retained.

You may object to processing based on legitimate interests. Contact us using the details above. You also have the right to complain to the Information Commissioner’s Office.
